If you are interested in becoming a new patient, please fill out the new patient forms and submit them to our office via mail, email, fax (802-275-7438) or dropping them off, and we will contact you after they have been processed.

We also collaborate with non-affiliated traditional or complementary practitioners in a patient’s current healthcare team. We can assist you in integrating those services and options that best meet your needs so you will experience continuity of care.
