We are excited to welcome you to our practice! To ensure we have all the necessary information prior to your first appointment, please take the following steps:
1. Update Your Medical Record

Please ensure your medical records are updated at least 24 hours before your appointment.
2. Submit Insurance Information
Email or fax us copies of the front and back of all your insurance cards. You can fax them to 802-275-4738 or email them to [email protected]. Please send these to us at least 24 hours before your appointment.
3. Review Consent Documents
Log into your patient portal and carefully review the consent documents before your first appointment: Patient Portal Link
